Hazardous Waste Manifest
EPA recently revised the Hazardous Waste Manifest Forms used to track hazardous waste from a generator's site to the site of its disposition. Handlers of waste must obtain forms from a source that has registered with US EPA to print and distribute the form. Please refer to EPA's Manifest Registry web page for a list of approved registered printers. Submission of Copies of Manifests
West Virginia Department of Environmental Protection does not require copies of manifests be submitted, regardless of whether the waste originates or is disposed of in the state of West Virginia.
- When the shipment is an international shipment, or
- Waste is being shipped utilizing a West Virginia state-issued one-time provisional US EPA ID Number. The provisional number is used for a one-time cleanup of regulated waste, and is usually issued for a period of 30 days.
